Entorno laboral y seguridad del paciente: comparación de datos entre los estudios SENECA y RN4CAST

Abstract

Resumen Objetivo Analizar la relacion entre el entorno laboral y el sindrome de burnout de los profesionales de Enfermeria con la calidad de los cuidados para la seguridad del paciente, presentados en los hospitales del Sistema Nacional de Salud coincidentes en los estudios SENECA y RN4CAST. Metodo Estudio descriptivo mediante el analisis secundario en el que se comparan datos de 984 historias de pacientes, 1.469 encuestas a pacientes y 1.886 encuestas a profesionales del estudio SENECA, con 2.139 encuestas a enfermeras del estudio RN4CAST en los 24 hospitales. Se compararon datos de eventos adversos relacionados con los cuidados, percepcion de la seguridad del paciente y de los profesionales, con el ambiente trabajo (medido mediante el Nursing Work Index) y el burnout (mediante el Maslach Burnout Inventory). Resultados Se observo una relacion estadisticamente significativa del dolor con la «Dotacion y adecuacion de recursos» (r = −0,435; p = 0,03) y la infeccion nosocomial con «Fundamentos enfermeros para unos cuidados de calidad» (r = −0,424; p = 0,04) y con «Participacion de la enfermera en cuestiones del hospital» (r = −0,516; p = 0,01) del Nursing Work Index. La clasificacion del hospital segun el Nursing Work Index se relaciona con la percepcion de seguridad por parte de los pacientes (r = 0,66; p La percepcion de la participacion por parte de los profesionales en la seguridad de los pacientes se ve relacionada con los 5 factores del Nursing Work Index (r ∈ [|0,41|-|0,78|]; p Conclusiones Las instituciones que fomenten un entorno laboral favorable tendran pacientes que perciban una mayor seguridad de los cuidados. Ademas, una adecuada gestion de los recursos podria disminuir la aparicion de eventos adversos como el dolor.
