Meningitis aséptica aguda: muchas causas a considerar

Abstract

La meningitis aseptica es un proceso de naturaleza fundamentalmente infecciosa que afecta a las envolturas o meninges del sistema nervioso central (SNC). El termino aseptico hace referencia a la ausencia de microorganismos en la tincion de Gram y negatividad de los cultivos habituales para agentes bacterianos en el liquido cefalorraquideo (LCR). Se caracteriza por fiebre, signos y sintomas meningeos (cefalea y rigidez de nuca), moderado incremento celular (habitualmente pleocitosis linfocitaria), valores de proteinas normales o discretamente elevados, glucosa en el rango de la normalidad y aspecto claro del LCR. Si la instauracion de las manifestaciones clinicas se produce de forma rapida, en horas o pocos dias, hablamos de meningitis aseptica aguda (MAA). Nos referimos a una meningitis aseptica cronica cuando los signos y sintomas de afectacion del SNC duran al menos 4 semanas, no son muy llamativos y, como en el caso de las MAA, no encontramos microorganismos mediante el estudio convencional del LCR. En este caso tambien deben constatarse datos de inflamacion cronica en el LCR. En ocasiones las MAA tienen un curso recidivante, como es el caso de la meningitis de Mollaret o las provocadas por virus herpes 21. Diferenciar entre las MAA, producidas en su mayoria por virus, generalmente de curso benigno y autolimitado, y las meningitis bacterianas agudas puede no ser facil en algunas ocasiones. Este hecho es especialmente preocupante cuando nos enfrentamos a una meningitis decapitada por el uso precoz de antibioticos. Los antecedentes y ambiente epidemiologico, la presencia de determinados signos clinicos junto a un analisis basico de pruebas de laboratorio (recuento y tipo celular, valores de glucosa, proteinas, ADA, VDRL, pruebas de determinacion rapida de antigenos y, ocasionalmente, otras determinaciones serologicas) han sido hasta hace pocos anos las herramientas que hemos manejado para el diagnostico y filiacion de las meningitis. En muy pocos centros se realizaba el cultivo de virus y solo la experiencia clinica guiada de las pruebas antes citadas permitia la toma de decisiones como la suspension del tratamiento antimicrobiano iniciado de forma empirica o un alta rapida de los pacientes. Las meningitis causadas por virus han recibido menos atencion por considerarse menos graves y de resolucion espontanea, y no existen estudios
