Queratitis fúngica en paciente inmunocompetente

Abstract

Paciente de 41 anos, que acude al Servicio de Oftalmologia para evision de lesion en ojo izquierdo, diagnosticada como ulcera heretica una semana antes en Urgencias y tratada con aciclovir y obramicina topicos. La lesioneradeaparicionespontanea, sin trauatismo previo; el paciente no era portador de lentes de contacto. omo antecedentes de interes, fue intervenido 12 anos antes de ueratectomia fotorefractiva miopica con laser excimer. Presenaba intenso edema corneal, con infiltrado paracentral de aspecto lanquecino. Se diagnostica queratitis bacteriana y se pauta traamiento con ceftazidima y vancomicina IV, ademas de colirios eforzados de estos mismos farmacos. Se obtuvo raspado corneal de la lesion y se inoculo en medios e agar sangre, agar chocolate y agar Sabouraud. A los 5 dias de ncubacion se obtuvo el crecimiento en los tresmedios de un hongo lamentoso en cultivo puro que al cabo de 15 dias de incubacion ostro el aspecto de la figura 1. Al examen microscopico con azul de lactofenol se observaron ifas y conidias hialinas; la incubacion prolongada (20 dias) peritio observar cleistotecias compatibles con Pseudallescheria spp. fig. 2).
