Étude par culture du microbiote urinaire en pédiatrie

Abstract

Introduction Le microbiote est l’ensemble des micro-organismes associe a un environnement. Il est le plus souvent etudie par des techniques de metagenomique, mais la culture peut permettre une approche complementaire. Les urines sont historiquement considerees steriles. Des etudes recentes ont prouve l’existence d’un microbiote urinaire en metagenomique et en culture. En pediatrie, une seule etude a etudie le microbiote urinaire, par des techniques de metagenomique sur des echantillons de premier jet urinaire. Objectif Decrire le microbiote urinaire par des techniques de culture speciales chez des patients presentant des pathologies renales et des temoins en pediatrie. Materiel et methodes Nous avons recueilli des echantillons urinaires mictionnels de milieu de jet apres desinfection perineale. Ces echantillons ont ete cultives sur 6 milieux de culture solides et 4 milieux de culture liquides, en condition d’aerobie et d’anaerobie. Les colonies bacteriennes ont ete identifiees par spectrometrie de masse (MALDI-TOF) puis sequencage de l’ADN codant pour l’ARN 16S si besoin. Resultats Les 12 echantillons recueillis ont permis d’identifier en moyenne 16,1 especes bacteriennes differentes par echantillon soit 98 bacteries differentes au total dont 38 n’ont jamais ete identifiees dans des urines humaines. Le metabolisme de ces bacteries etait l’anaerobie dans 32,6 % des cas. Le nombre d’especes isolees etait significativement plus important chez les individus de sexe feminin. Discussion/conclusion Il existe un microbiote urinaire en pediatrie qui peut etre decrit par des techniques de cultures speciales. La standardisation de ces techniques permettrait une approche complementaire a la metagenomique pour decrire le microbiote urinaire et comprendre son role en physiologie et en pathologie.
