
Resume Objectif L’invagination intestinale postoperatoire est consideree comme une complication frequente des gestes operatoires abdominaux. Cas clinique Une enfant de 24 mois etait hospitalisee dans un tableau de vomissements et de pleurs associes a une hyperleucocytose neutrophile, sans signes physiques abdominaux. La radiographie du thorax mettait en evidence la presence d’anses intestinales dans l’hemithorax gauche. L’exploration chirurgicale permettait de reconnaitre et traiter une volumineuse hernie de Bochdalek interessant une partie du colon gauche et le colon transverse. A la 7 e journee postoperatoire, le tableau clinique se compliquait d’une occlusion intestinale avec deterioration de l’etat general. L’exploration chirurgicale mettait en evidence une invagination ileo-ileale a 15 cm de la valve ileocaecale, en l’absence de point de traction . Conclusion L’invagination intestinale peut compliquer le traitement d’une hernie diaphragmatique et s’expliquer par l’atonie residuelle du colon transverse une fois reduit dans l’abdomen. Cette atonie servirait de leading point fonctionnel pour l’ileon d’amont durant la phase de reactivation du peristaltisme intestinal dans les premiers jours postoperatoires.
