Hiperplasia hipofisaria secundaria a hipotiroidismo

Abstract

Nina de 10 anos 4 meses con hallazgo de masa hipofisaria (1,7 1,5 1,5cm) con impronta quiasmatica en control de imagen de quiste aracnoideo (fig. 1). Hipocrecimiento en los ultimos 2 anos. Antecedentes de hipotiroidismo en rama paterna. Exploracion fisica: talla 129,5cm ( 1,87DS), IMC 19,28kg/m (þ0,12DS). FC 58 lpm. Tanner I. Resto normal. Analitica sanguinea normal, salvo trigliceridos 132mg/dl, colesterol total 228mg/dl (LDL 132, HDL 69). Osmolaridad urinaria y plasmatica normales. Estudio hormonal: PRL basal 43,35ng/ml (0–18,5), IGF-1 48,56ng/mdl (76–499). Hormonas tiroideas: T3 total 0,63ng/ml (0,6–1,9), T3 libre 1,01pg/ml (2–4,25), T4 total 23,27ng/ml (45–110), T4 libre 1,21pg/ml (7–18), TSH 42,24mUI/ml (0,25–5), tiroglobulina 84,49ng/ml (0–75). Ac antitiroglobulina 17,56U/ml, Ac antiTPO 246,06 (0–130). LH, FSH, cortisol y ACTH normales. Edad osea: 9 anos (TWRUSII). Ecografia tiroidea: aumento de tamano glandular con ecogenicidad alterada. Con el diagnostico de tiroiditis de Hashimoto e hiperplasia hipofisaria secundaria, se inicia tratamiento con levotiroxina con normalizacion de TSH y hormonas tiroideas, IGF-1, PRL, y perfil lipidico, asi como catchup de talla. En resonancia magnetica (RM) de control a los 5 meses se objetiva normalidad de region hipofisaria (fig. 2). La evolucion prolongada de un hipotiroidismo sin tratamiento puede originar una hiperplasia de celulas tirotropas, resultando indistinguible en ocasiones de un macroadenoma hipofisario en la RM. Puede existir hiperprolactinemia por exceso compensatorio de TRH y/o compresion de tallo. El diagnostico diferencial es muy importante ya que permite evitar conductas terapeuticas agresivas.
