Une forme historique de maladie de Von Recklinghausen

Abstract

Introduction La maladie de Von Recklinghausen ou neurofibromatose de type 1 (NF1) est une maladie genetique dont la prevalence est de 1/2500 naissances. Son association a un pheochromocytome varie de 0,5 a 5 %. Ce dernier est le plus souvent asymptomatique. Nous rapportons une forme historique peu habituelle associee a un pheochromocytome tres symptomatique. Cas clinique Patient âge 45 ans aux antecedents familiaux de NF1 (3 membres) consulte pour HTA avec sueurs, palpitations et douleurs basi-thoraciques, evoquant un pheochromocytome. Il presente des taches « cafe au lait », des lentigines axillaires, des nodules de Lisch et des neurofibromes cutanes dissemines dont le plus volumineux mesure 20 cm. La biologie retrouve une elevation importante des derives methoxyles : normetanephrines = 3750nmol/24 h (44–213), metanephrines : 1665 nmol/24 h (40–228) et de la chromagranine A : 1003 ng/mL (20–115). La TDM thoraco–abdomo–pelvienne retrouve une masse surrenalienne droite de 64 × 62 × 45 mm, ovalaire et bien limitee, discretement hypodense, se rehaussant de facon heterogene apres injection du produit de contraste. La scintigraphie a la MIBG etait en faveur d’un processus neuro-ectodermique. Par ailleurs, il avait des neurofibromes au niveau mediastinal posterieur, pleural, et du psoas droit. La TDM cerebrale etait normale. L’echo doppler des arteres renales ne montrait pas de stenose. L’echographie thyroidienne et le thyrocalcitonine etaient normales. Ce patient est adresse en chirurgie pour exerese du pheochromocytome et du volumineux neurofibrome avec etude anatomopathologie. Conclusion Cette observation se distingue par l’aspect historique du tableau clinique de la NF1 et le caractere tres symptomatique du pheochromocytome qui lui est associe.
