Abstract

Introduction L’ovotestis est une cause rare d’ambiguite sexuelle caracterisee par la presence chez un patient de tissu testiculaire et ovarien, conduisant au developpement de structures masculines et feminines. Observation Enfant âge de 15 ans. L’examen a 2 mois de vie retrouve un bourgeon genital de 3 cm, un hypospadias posterieur et deux bourrelets de type feminin. L’echographie pelvienne ne retrouve pas de testicules, ni d’uterus. Le bilan biologique avant test au pregnyl : FSH a 2,49 mui/mL (1 a 12), LH a 4,92 mui/mL (2 a 12) et testosteronemie a 0,16 ng/mL (0,1 a 0,3) ; Apres test, on note une augmentation de la testosteronemie a 5,6 ng/mL temoignant d’une fonction testiculaire preserve. Le caryotype est de type masculin 46 XY. L’exploration laparoscopique a montre a droite un pedicule spermatique et un deferent convergeant vers le canal inguinal droit qui est ouvert. A gauche : il existe un vestige mullerien avec presence d’une gonade evoquant un ovaire. Le geste chirurgical a permis la resection du vestige mullerien a gauche ; a droite, cure de l’hernie avec abaissement du testicule droit. A l’anapath, il s’agit d’un tissu fibreux qui renferme des structures tubaires, ovariennes et testiculaires (ovotestis). Discussion/conclusion La prise en charge de cette pathologie est delicate. Si le diagnostic est pose a la naissance, la decision concernant le choix du sexe dans lequel sera eleve l’enfant doit etre discutee en fonction de l’aspect des organes genitaux et de l’histologie des gonades. L’ablation du tissu correspondant au sexe oppose et une hormonotherapie substitutive appropriee doivent etre proposee.
