Hungry bone syndrome : à propos d’un cas

Abstract

Introduction Le Hungry bone syndrome (HBS) est une cause rare mais redoutable de l’hypocalcemie persistante apres chirurgie curative d’une hyperparathyroidie severe. L’HBS est marque par une hypocalcemie profonde avec hypophosphoremie necessitant ainsi un traitement precoce et adequat. Nous en rapportons un cas. Observation L’hyperparathyroidie de notre patiente de 63 ans a ete diagnostiquee devant des myalgies diffuses, des douleurs osseuses et une asthenie. Le bilan biologique et morphologique a objective une hypercalcemie a 131 mg/L avec une PTH a 857 ng/L et un adenome parathyroidien inferieur gauche. Les repercussions osseuses etaient manifestes. La patiente a beneficie en preoperatoire d’une hydratation intraveineuse et d’une cure de vitamine D devant une carence. En postoperatoire, une hypocalcemie severe a 57 mg/L s’est installee avec un taux de PTH restant bas a 11 pg/mL. Cependant, la supplementation majeure en calcium et 1,25 vitamine D avec une calciurie effondree, des PAL elevees et une phosphoremie basse ont fait retenir le diagnostic de HBS associe a une hypoparathyroidie. Discussion L’HBS est le resultat d’une avidite exacerbee de l’os pour le calcium qu’il faudra redouter devant toute hyperparathyroidie severe, pouvant etre prevenu par un traitement tres precoce car sa prise en charge reste difficile et prolongee faisant appel a de fortes doses de calcium et de vitamine D.
