Évaluation du niveau de connaissance de la maladie chez les diabétiques de type 1

Abstract

Objectif Evaluer le niveau d’education des diabetiques de type 1 au sein de la consultation. Patients et methode Chez 100 patients âges de 15 a 50 ans, diabetiques de type 1 depuis au moins 2 ans, nous avons evalue le niveau de connaissance de la maladie. Resultats Quatre-vingt-neuf pour cent des patients avouent ne pas etre encore « habitues » a la maladie. Soixante-dix pour cent seulement sont sous schema optimise basal-bolus. Treize pour cent des patients oublient au moins une fois par semaine leur injection d’insuline. Cinquante et un pour cent des patients savent qu’ils n’observent pas les regles dietetiques notamment concernant le pain les fruits et les boissons. Vingt-deux pour cent n’observent pas l’autosurveillance. Seuls 24 % pratiquent plus test par jour. L’autocontrole est celle-ci est parfois aleatoire devant des signes cliniques d’hyperglycemie, anticipatoire devant un repas riche en hydrate de carbone sans tenir compte toutefois des resultats de la glycemie capillaire 66 % des patients n’ont une notion sur le glucagon. Trente-trois pourcent des patients ont eu au moins 2 hospitalisations dues a la maladie. Malgre les conseils de leurs medecins, 67 % ont pratique le jeune pendant au moins 10 jours avec des hypoglycemies frequentes. Conclusion Le diabete de type 1 est une pathologie lourde du fait de la contrainte de la multiple injection et l’autosurveillance pluriquotidienne qu’il impose. De ce fait l’adhesion au traitement est difficile. L’education therapeutique permet en grande partie d’ameliorer l’observance therapeutique et l’equilibre glycemique.
