IgE totales et IgE spécifiques des aliments de l'enfant (Rast F×5®) au cours de la croissance de l'enfant

Abstract

Resume But. – Le but de cette etude transversale etait de decrire l'evolution des IgE seriques totales et du Rast F×5® sur la base d'un dosage systematique chez les enfants consultant pour la premiere fois en allergie alimentaire. Patients et methodes. – Neuf cent vingt-cinq enfants d'âge moyen 8,9 ± 9,1 ans (15 jours–18 ans), 455 filles et 470 garcons (annee 1997), dosage serique des IgE seriques totales et specifiques pour six aliments (proteines du lait de vache, blanc d'œuf, arachide, soja, ble, poisson) groupes dans le Rast F×5 (Cap System®, Pharmacia and Upjohn Diagnostics AB®, Uppsala, Suede) et analyse en fonction des valeurs de reference normales pour l'âge. Resultats. – Le titre des IgE seriques totales augmente significativement avec l'âge, r = 0,22 p = 0,0001. Le pourcentage d'enfants presentant un taux d'IgE seriques totales au-dessus de la valeur normale pour l'âge augmente avec l'âge (10,3 % a 0–6 mois, 38,2 % a 1–2 ans), atteint un pic a 4–6 ans (56,9 %, p 12 ans, ns). Le pourcentage d'enfants presentant des IgE specifiques groupees (Rast F×5®) detectables pour l'âge augmente de maniere parallele jusqu'a 4–6 ans, puis diminue nettement a partir de 6–8 ans par rapport au pourcentage d'enfants dont les IgE totales sont elevees : 31,9 vs 52,9 %, p 12 ans). Conclusion. – Dans cette population d'enfants consultant pour une suspicion d'allergie alimentaire, l'elevation avec l'âge du pourcentage d'IgE seriques totales elevees contraste avec la decroissance progressive de celui des IgE specifiques pour les aliments de l'enfant observee a partir de 4–6 ans. Cette observation peut etre rapprochee de l'acquisition progressive de la tolerance alimentaire au-dela de la petite enfance.
