Abstract

Resumen Introduccion La ventilacion mecanica invasiva suele suscitar dilemas-eticos en pacientes con enfermedad cronica/terminal en insuficiencia respiratoria aguda grave. El conocimiento cientifico, y aplicacion de los valores humanos son necesarios en la toma de decisiones. Objetivo Presentar la Bioetica a traves del conocimiento cientifico, y los valores humanos que deben ser aplicados en pacientes con enfermedad cronica/terminal en insuficiencia respiratoria aguda grave que requieren de ventilacion mecanica invasiva. Metodologia Se realizo una revision no sistematizada de la literatura medica relacionada con las enfermedades cronicas/terminales que ocasionan insuficiencia respiratoria aguda grave, y que requieren de ventilacion mecanica invasiva. La Bioetica se reviso con base en el conocimiento cientifico (bios) de la ventilacion mecanica invasiva, y los valores humanos (etica) con base en derechos y principios eticos. Resultados Las enfermedades cronicas/terminales como la enfermedad pulmonar obstructiva cronica, la insuficiencia cardiaca congestiva cronica, la enfermedad cerebrovascular, el sindrome de inmunodeficiencia, el cancer y la esclerosis lateral amiotrofica, ocasionan insuficiencia respiratoria aguda grave, que requiere apoyo de la ventilacion mecanica invasiva, y generando dilemas-eticos en cuanto a iniciar o no la ventilacion mecanica. La decision etica y moral de iniciar o no iniciar la ventilacion mecanica se apoya en derechos del paciente, principio de autonomia, principio de beneficencia, principio de proporcionalidad y el fin previsible del paciente. Analisis estadistico La metodologia de investigacion ha sido analitica, descriptiva, no requirio de estadistico. Comentario La Bioetica no interviene en la fisiopatologia respiratoria de las enfermedades cronicas/terminales en insuficiencia respiratoria aguda grave. Tampoco en los conceptos basicos de la ventilacion mecanica invasiva. La Bioetica interviene en la responsabilidad del conocimiento cientifico y el reconocimiento de los valores humanos en el acto medico.
