Abstract

Resumen Segun la Organizacion Mundial de la Salud cada ano mueren cerca de 265.000 personas a causa de quemaduras, principalmente en paises de bajos ingresos. Una quemadura se define como la lesion aguda de la piel u otro tejido causada por un cambio termico que concluye en la lesion total o parcial de las celulas, se puede producir por liquidos o solidos, calientes o frios, llama, radiacion, electricidad, friccion o contacto con agentes quimicos. Estas quemaduras se clasifican segun la profundidad, el compromiso tisular y la superficie corporal afectada con el fin de definir el manejo, pronosticar la mortalidad, evitar posibles complicaciones asociadas y determinar la necesidad de seguimiento en unidades de alta dependencia. Una quemadura mayor se define como aquella que compromete 25% o mas de la superficie corporal total. El tratamiento inicial de estos pacientes debe ser en la Unidad de Cuidados Intensivos y esta orientado a la estabilizacion inicial, con un adecuado soporte ventilatorio, reanimacion hidrica, estabilizacion cardiovascular, control del dolor y manejo local de las heridas. El fin del manejo es mantener la perfusion, prevenir el choque, la falla organica multiple y evitar la morbimortalidad asociada a la sobrerreanimacion, el sindrome compartimental abdominal, la rabdomiolisis, la lesion renal aguda y las infecciones.
