Abstract

Resumen El sindrome de shock toxico asociado a superantigenos pone en marcha una respuesta sistemica que determina su caracterizacion clinica. Presentamos un caso en el Hospital Susana Lopez de Valencia que cumple los criterios diagnosticos del sindrome de shock toxico al manifestar una temperatura > 38,9 °C, exantema, hipotension arterial y afectacion de 2 o mas organos. Ahora bien, al considerar celulitis y bacteriemia por Staphylococcus aureus, asi como la posible coinfeccion estreptococica, se indico tratamiento con clindamicina, vancomicina y ceftriaxona, asociado posteriormente a inmunoglobulina intravenosa por 3 dias ante la sospecha de sindrome de shock toxico, presentando una evolucion clinica favorable, sin secuelas ulteriores. Lo anterior evidencia la importancia de la sospecha clinica para establecer un tratamiento antibiotico y terapia de soporte eficaz con rapidez.
