Neuromonitorización hemodinámica por ultrasonido en el paciente crítico: ultrasonido transcraneal

Abstract

Resumen El ultrasonido transcraneal (UTC) es una herramienta que desde 1982 ha ido ganando terreno en el area de la monitorizacion de la hemodinamica cerebrovascular. En el presente trabajo se realizo una revision de la literatura cientifica relacionada con el UTC y su utilizacion clinica en el area de terapia intensiva. Para su realizacion es necesario un transductor de baja frecuencia el cual se coloca en diferentes puntos anatomicos del craneo permitiendo la evaluacion de las arterias cerebrales y midiendo diferentes variables hemodinamicas: 1) la velocidad de los flujos cerebrales; 2) estimacion de la resistencia cerebral vascular; y 3) la presion de perfusion cerebral. A traves de estas y el analisis de las ondas de velocidad, el UTC puede ser utilizado en la terapia intensiva para la monitorizacion de eventos cerebrales isquemicos o hemorragicos, intra- y postoperatorios de bypass coronario, intra- y postoperatorio neuroquirurgico, neuromonitorizacion del paciente con trauma craneoencefalico y confirmacion diagnostica de muerte cerebral. En esta revision se describen los principios fisicos del UTC, tecnica, medicion de las velocidades e indices mas utilizados en la terapia intensiva, sus indicaciones y finalmente las limitaciones de esta herramienta en los Cuidados Criticos.
