Porfirias agudas: enfoque diagnóstico y terapéutico

Abstract

Resumen Las porfirias agudas son trastornos congenitos poco frecuentes debidos a alteraciones en las enzimas de la biosintesis del grupo hem. Estas enzimopatias pueden ser de caracter autosomico dominante (porfiria aguda intermitente, coproporfiria hereditaria, porfiria variegate) y de caracter autosomico recesivo (porfiria por deficiencia de acido delta-aminolevulinico dehidratasa). Tradicionalmente las porfirias han sido clasificadas en hepaticas o eritropoyeticas segun el sitio primario de sobreproduccion. La importancia clinica de este tipo de enfermedades en los servicios de emergencia y unidades de cuidado intensivo radica, sobre todo, en los llamados ataques agudos de porfiria, en especial, el ataque agudo de la porfiria intermitente aguda, la cual se caracteriza por una serie de signos y sintomas inespecificos, entre los que el dolor abdominal de caracteristicas agudas es el sintoma principal de presentacion. Se debe prestar atencion a los factores precipitantes de los ataques agudos por la elevada morbimortalidad. Hacer un diagnostico rapido y oportuno por medios cualitativos y cuantitativos es de vital importancia, solicitando porfobilinogeno y acido delta aminolevulinico. El tratamiento se basa principalmente en evitar factores precipitantes, iniciar de inmediato la terapia intravenosa con derivados hematinicos (Normosang®, Orphan Europe SARL, Puteaux, Francia) y en Norteamerica como hematina liofilizada (Panhematin®, Ovation Pharmaceuticals Inc., Deerfield, IL, EE. UU.), manejo de las complicaciones con los diferentes medicamentos avalados para su tratamiento. El pronostico es bueno si la condicion es diagnosticada y tratada de forma temprana con agresividad.
