¿Los gases venosos periféricos pueden reemplazar a los gases arteriales en la evaluación de pacientes críticos?

Abstract

Resumen La gasometria arterial es un estudio que se realiza en forma rutinaria para los pacientes enfermos y que arroja mucha informacion a la hora de tomar decisiones, pero a traves del tiempo se ha tomado como referencia para esta los gases arteriales, los cuales muchas veces son dificiles de tomar, son dolorosos y es tecnicamente exigente teniendo como consecuencia muchas veces el dano de la pared arterial, sobre todo cuando se requiere de mediciones repetidas; la oximetria de pulso es una alternativa no invasiva diferente a los gases arteriales pero no permite la evaluacion de la presion parcial de dioxido de carbono (pCO2). Teniendo en cuenta la toma de muestras rutinaria en los servicios de hospitalizacion y que muchas veces se hace mas facil tomar una muestra de sangre venosa en la cual podriamos medir pH, pCO2, pO2, niveles de bicarbonato y lactato, se ha realizado una revision de diferentes estudios que comparan gases arteriales y gases venosos en escenarios multiples, para determinar la importancia de estos y si en realidad los gases venosos perifericos podrian reemplazar en algun momento a los gases arteriales dando como resultado que estos pueden ser equiparables en las estimaciones del pH. Por el momento, y mientras no aparezcan grandes estudios que demuestren lo contrario, los gases venosos no pueden reemplazar los gases arteriales en el analisis acido-base y de oxidacion en pacientes criticos.
