Abstract

Resumen Objetivo Describir el comportamiento de las variables PiCCO en pacientes con choque refractario de cualquier etiologia, su desempeno para diferenciar los tipos de choque y mortalidad asociada. Diseno Estudio de cohorte prospectivo. Marco de referencia Unidad de cuidados intensivos, Hospital La Divina Misericordia, Magangue, Bolivar (Colombia). Pacientes Choque refractario de cualquier etiologia que requirieron monitorizacion con sistema PiCCO. Mediciones Se establecieron 2 tipos de choque (septico y no septico). Se realizo analisis para muestras parametricas y no parametricas y de regresion logistica. La capacidad de discriminacion se evaluo mediante curva ROC. Resultados Se analizaron 47 pacientes. Los pertenecientes al grupo sobreviviente eran significativamente mas jovenes 33,5 (18,75-64) vs. 68 (44-78,5) anos y tuvieron una estancia en UCI mas prolongada 18,5 (9,75-44,5) vs. 5 (3-8) dias. Las variables PiCCO gasto cardiaco, resistencias vasculares, fraccion de eyeccion global, indice de funcion cardiaca, poder cardiaco fueron significativamente diferentes para categorizar los diferentes tipos de choque. La mortalidad fue del 78,7% (n = 37), sin diferencias en el APACHE II entre los grupos. Las variables de contractilidad volumen sistolico, volumen sistolico indexado, fraccion de eyeccion global, poder cardiaco, indice de funcion cardiaca, se relacionaron con mortalidad, con una buena discriminacion (AUC 0,78-0,79). Conclusiones Las variables PiCCO relacionadas con contractilidad diferencian los tipos de choque refractario. Estas variables en el contexto de un estado hipodinamico se asocian con mortalidad en este escenario de choque.
