Abstract

Resumen El lactato es el producto final del metabolismo anaerobio y una valiosa herramienta diagnostica para la interpretacion, estratificacion y comprension de diferentes procesos patologicos del paciente criticamente enfermo. Gracias al avance tecnologico, que ha permitido un crecimiento constante en las tecnicas de laboratorio, el lactato tiene hoy en dia una amplia aplicabilidad en la practica clinica, permitiendo un criterio medico mas objetivo con el fin de establecer riesgos, diagnosticos, pronosticos y guiar tratamientos encaminados a mejorar la perfusion. Como todo examen diagnostico, la interpretacion del lactato requiere un conocimiento de los procesos fisiopatologicos y de las variables de confusion, que resulte en una toma adecuada de decisiones clinicas generando un beneficio para el paciente. En esta revision se ha realizado una busqueda exhaustiva de la literatura con el objetivo de aclarar el papel del lactato como biomarcador de homeostasis. Se hace una descripcion del origen, metabolismo, aclaramiento y utilidad del lactato, demostrando las variables mas importantes que pueden afectar su valor e interpretacion como prueba paraclinica, y ofrece una guia que facilita la interpretacion del mismo y su correlacion con otros biomarcadores disponibles para el tratamiento del paciente criticamente enfermo.
