Abstract

Resumen Uno de los objetivos fundamentales en el enfoque inicial del paciente critico es la valoracion y la manipulacion del sistema cardiovascular para asegurar un adecuado aporte tisular de oxigeno y sustratos metabolicos esenciales. En este sentido, la monitorizacion de parametros fisiologicos en la unidad de cuidado intensivo y en pacientes de alto riesgo quirurgico en salas de cirugia, resultan imprescindibles para lograr una terapia hidrica y un soporte vasopresor e inotropico racional y dirigido a metas terapeuticas tempranas. La hipovolemia es muy frecuente en pacientes en estado de choque o de falla circulatoria aguda y los liquidos por via intravenosa son generalmente la primera intervencion en la reanimacion inicial. Sin embargo, estudios clinicos han demostrado que aproximadamente solo el 50% de esos pacientes son respondedores a retos de liquidos. Es decir, que aumentan el volumen sistolico o el gasto cardiaco en respuesta a una carga de liquidos. Por otro lado, volumenes excesivos de liquidos tienen consecuencias negativas, incluyendo: edema intersticial, alteracion en la transferencia de oxigeno, disminucion de la relajacion miocardica, ventilacion mecanica prolongada, altas estancias en UCI, aumento de mortalidad y de costos. El cateter de arteria pulmonar esta siendo reemplazado por monitorizacion hemodinamica menos invasiva, mas simple de utilizar y mas precisa para predecir respuesta a la terapia hidrica, vasopresora e inotropica. Estas tecnologias son mas costo-efectivas, de tal forma que se pueda monitorizar la precarga cardiaca, contractilidad cardiaca, poscarga, agua pulmonar extravascular y de esta manera dirigir el tratamiento para mejorar el desenlace de los pacientes en terminos de menos complicaciones postoperatorias, menor duracion de ventilacion mecanica, menos estancia en la unidad de cuidado intensivo y hospitalaria.
