Intoxicación aguda por carbamatos durante el embarazo

Abstract

Resumen Introduccion Los intentos de suicidios y envenenamientos en el embarazo son un reto para los profesionales de la salud debido a los efectos desconocidos del agente toxico y la terapia antidoto contra los fetos; ademas que dichos agentes pueden tener un comportamiento diferente derivado de los cambios fisiologicos de la gestacion que la hacen mas susceptible a determinados toxicos o medicamentos y sus acciones especificas sobre el bienestar del binomio materno fetal. Caso clinico Paciente femenina de 26 anos de edad, con embarazo de 20 semanas, no controlado, quien ingiere con fines autolesivos 20 g de aldicarb (carbamato), con posterior presentacion de dolor abdominal de predominio epigastrico, nauseas, disnea, sialorrea y posteriormente somnolencia, bradicardia e hipotension severa, que recibio manejo con fluidoterapia, lavado gastrico, una dosis de atropina y monitoreo en cuidados intensivos por 24 horas, con evolucion clinica satisfactoria y alta 72 horas posterior al ingreso, con adecuada evolucion de gestacion. Discusion En la intoxicacion por Carbamatos los sintomas se presentan derivados de una inhibicion reversible de la acetilcolinesterasas, que da origen a sintomas muscarinicos y nicotinicos de corta duracion; en este tipo de intoxicaciones el feto merece la mejor atencion y seguimiento ecografico aunque la madre recibe un tratamiento similar al de mujeres no embarazadas pues no hay evidencia que contraindique el uso del mismo esquema de desintoxicacion y terapia de apoyo en la gestante. No existe otro caso reportado en la literatura de intoxicacion aguda por carbamatos durante el embarazo, razon que motivo el reporte del mismo.
