</script>We present a rare case of uterine rupture in a 16year-old primigravida who was admitted complaining of abdominal pain and mild vaginal bleeding. Findings on examination were: height of fundus 30 weeks,. absence of fetal heart sounds, cervix 4-5 cm dilated, intact membranes, and head O/5 above the pelvic brim. The patient was admitted to the labor ward with a diagnosis of intrauterine fetal death in labor. Six hours later, artificial rupture of the membranes was carried out and an infusion of 2.5 IU/l followed by 10 IU oxytocin synthetic was commenced. Ten hours later the cervix dilated to 9 cm accompanied by very mild contractions. Oxytocin synthetic 20 IU, sedation and antibiotics were added. Two hours later the cervix was still 9 cm and the head was 3 cm below the ischial spines. Vacuum extraction was carried out and the head came out without difficulty but with arrested shoulders and difficulty in rotating the body (only elongated neck felt on vaginal examination).
