Abstract A case is reported of encephalitis of undetermined etiology which led to severe hyperelectrolytemia and hyperosmolarity of the extracellular fluid. The causes of this condition were a primary disturbance of the central osmoregulatory mechanism, a lack of antidiuretic hormone secretion and a prolonged diuresis of relatively dilute urine. Therapy consisted of rehydration by fluids low in electrolytes.
