Abstract

Hintergrund und Fragestellung. Die Inzidenz maligner Tumoren des Kopf-Hals-Bereichs steigt weltweit an. Hauptrisikofaktoren sind der chronische Tabak- und Alkoholabusus. Durch die Erkennung von Hochrisikopatienten konnen fruhzeitig diagnostizierte Tumoren mit guten Heilungsaussichten behandelt werden. Biomonitoringprogramme konnen wertvolle Aussagen zur individuellen Belastungs- und Gefahrdungssituation von Risikogruppen liefern. Durch den Einsatz des Ames-Tests als etabliertem gentoxikologischen Prufverfahren soll die Rolle des Speichels in der Kanzerogenese und seine Verwertbarkeit als Biomarker eingeschatzt werden. Patienten und Methodik. Speichelproben von 131 mannlichen Probanden wurden im Ames-Test auf gentoxische Effekte untersucht. Es erfolgte der Vergleich der Ergebnisse mit den Alkohol- und Rauchgewohnheiten der Probanden. Ergebnisse. Unter Rauchern ist die Zahl gentoxischer Speichelproben im Trend erhoht. Rauchen und Alkohol in Kombination fuhren zu einer hochsignifikanten Steigerung der gentoxischen Aktivitat im Speichel (p<0,002). Schlussfolgerung. Der Ames-Test kann ggf. in Zukunft in Kombination mit anderen Biomarkern zur Entwicklung eines tragfahigen Konzepts zur Erkennung von Hochrisikopatienten in epidemiologisch charakterisierten Risikogruppen beitragen. Eine prospektive Absicherung der Ergebnisse ist zuvor durch die Validierung des Ames-Test als Biomarker an Speichelproben von Tumorpatienten erforderlich.
