Abstract

Der endotracheale Tubus (ETT) ist eine atemmechanische Last, die stark flussabhangig und deshalb sehr variabel ist. Konventionelle Verfahren der Atemunterstutzung konnen den Tubuswiderstand in Inspiration nicht adaquat und in Exspiration uberhaupt nicht kompensieren. Automatische Tubuskompensation (ATC) kompensiert den flussabhangigen Druckabfall uber dem endotrachealen Tubus durch eine positive Druckunterstutzung in Inspiration und eine negative Druckunterstutzung in Exspiration. Die Druckunterstutzung folgt der nichtlinearen Druck-Fluss Kennlinie des ETT. ATC beruht auf einem Closed-loop-Prinzip, das indirekt “arbeitet”, da der Istwert des Trachealdrucks nicht direkt gemessen wird, sondern auf der Basis des kontinuierlich gemessenen Atemwegsdrucks und des Gasflusses berechnet wird. Dabei ist ATC kein eigenstandiger Modus der Atemunterstutzung, sondern vielmehr eine Komponente, die mit allen konventionellen Unterstutzungsmodi kombiniert werden kann. ATC liefert die Basis fur eine objektive Aufteilung der Druckunterstutzung, die zum einen durch den Tubus und zum anderen durch das respiratorische System notwendig wird. Partielle Tubusobstruktion, welche die Effektivitat von ATC vermindert, kann durch Analyse des exspiratorischen Fluss-Signals nichtinvasiv und automatisch detektiert werden. Eine entsprechende Software kann einfach in ein Beatmungsgerat implementiert werden. Die Effektivitat von ATC im Langzeiteinsatz kann durch intermittierende kurzzeitige Messung des Trachealdrucks gewahrleistet werden. Bisher ist kein kommerzielles Beatmungsgerat mit vollstandigem exspiratorischem ATC verfugbar. Studien an Probanden und tracheal intubierten Patienten haben auf uberzeugende Weise gezeigt, dass ATC die Atemarbeit reduziert und den subjektiven Atemkomfort erhoht. Zusatzlich konnte der Extubationserfolg unter ATC bei schwierig zu entwohnenden Patienten besser vorhergesagt werden als auf der Basis anderer Modi. Es gibt keine detaillierten Einstellregeln fur die klinische Anwendung von ATC. Um jedoch ein “overassist” zu vermeiden, sollte das Unterstutzungsniveau des in Kombination mit ATC gewahlten Modus reduziert werden.
