Meccanismo d’azione degli inibitori del sistema renina-angiotensina

Abstract

Gli inibitori del sistema renina-angiotensina comprendono un vasto gruppo di farmaci che esercitano la loro azione come inibitori dell’Angiotensin Converting Enzyme (ACE-inibitori), come antagonisti del recettore AT1 dell’angiotensina II o come inibitori della renina. Questi farmaci sono ampiamente utilizzati per la terapia dell’ipertensione arteriosa, ma costituiscono anche l’unica arma, apparentemente totipotente, per arrestare quella serie di eventi patologici che conduce alla sclerosi renale. I benefici effetti degli inibitori del sistema renina-angiotensina si manifestano indipendentemente dall’eziologia della sclerosi renale: scompenso metabolico del diabete mellito, ipertensione arteriosa, effetti tossici della ciclosporina nei reni trapiantati. A seconda del punto d’azione questi farmaci dimostrano una potenza inibitoria diversa sul sistema reninaangiotensina. Appare quindi utile riassumere le conoscenze sul meccanismo d’azione degli inibitori del sistema reninaangiotensina, focalizzando l’attenzione sulle principali differenze biologiche e cliniche che esistono tra i vari farmaci di questo gruppo.
