Perfil clínico de Exubera® en el tratamiento de la diabetes

Abstract

El tratamiento intensivo con insulina es un elemento esencial para alcanzar y mantener un buen control metabolico. La reduccion de la HbA1c es fundamental en la prevencion de las complicaciones cronicas de la diabetes. Sin embargo, muchos pacientes rechazan el inicio de la insulinizacion o la intensificacion del tratamiento insulinico por miedo a las inyecciones. La insulina inhalada es un nuevo metodo minimamente invasivo de administracion de insulina. La insulina inhalada es una insulina de accion rapida y debe administrarse antes de la comidas. En los pacientes con diabetes tipo 1 y tipo 2, la insulina inhalada es tan efectiva como la insulina regular en combinacion con diversas insulinas basales. En la diabetes tipo 2, la insulina inhalada de forma aislada o combinada con uno (sulfonilurea o metformina) o dos hipoglucemiantes orales ha demostrado ser muy efectiva, especialmente en aquellos pacientes con HbA1c basal elevada. Ademas, la INH ha demostrado un reduccion consistente de la glucemia en ayunas de causa desconocida. La insulina inhalada no incrementa el riesgo de hipoglucemia, los efectos pulmonares son leves y reversibles, y el incremento observado de anticuerpos antiinsulina no parece tener efectos adversos conocidos. El grado de satisfaccion general del paciente fue mayor con esta nueva insulina que con la insulina subcutanea. En resumen, la insulina inhalada (Exubera®), como nuevo metodo de administracion de insulina prandial, puede ser una valiosa alternativa de tratamiento en pacientes con diabetes tipo 1 y tipo 2, especialmente en aquellos resistentes al inicio o a la intensificacion del tratamiento insulinico como forma de alcanzar los objetivos de control.
