Anemie in de zwangerschap

Abstract

In 2000 publiceerde de knov de standaard Anemie in de eerstelijns verloskundige praktijk. De belangrijkste aanbevelingen van de standaard zijn het hanteren van lagere, aan zwangerschapsduur gerelateerde Hb-afkappunten bij zwangeren en het gebruik van de HB-bepaling als screeningstest in plaats van als diagnostische test. Bij een Hb onder de grenswaarde is verdere diagnostiek noodzakelijk om te differentieren tussen hemodilutie of anemie. De onderbouwing van deze aanbevelingen wordt enerzijds gevormd door inzicht in de fysiologische veranderingen van het bloed in de zwangerschap: hemodilutie die verandert in de loop van de zwangerschap. Anderzijds door de wetenschap dat lagere Hb-waarden niet gerelateerd zijn aan slechtere uitkomsten voor moeder en kind. Dit beleid leidt tot beduidend minder ijzerprescriptie.
