Glyburide of insulinebij zwangerschapsdiabetes?

Abstract

Twee tot vijf procent van de zwangerschappen wordt gecompliceerd door diabetes mellitus. Dit geeft aanleiding tot een aanzienlijke morbiditeit en soms sterfte bij moeder en kind. Er wordt meestal een behandeling met insuline ingesteld. Er zijn echter aanwijzingen dat het sulfonylureumderivaat glyburide een goed alternatief is. Jacobson en medewerkers doen verslag van een onderzoek, waarbij retrospectief de resultaten van behandeling met glyburide (n=236) met die met insuline (n=268) werden vergeleken1. Bij de zwangeren was de diabetes met behulp van een standaard glucosetolerantietest vastgesteld bij een zwangerschapsduur van 12 tot 34 weken. De patienten moesten een dieet houden en zelf viermaal per dag een bloedsuikerbepaling doen. De dosis glyburide werd op geleide van de bloedsuikerbepalingen opgevoerd van minimaal 2,5 mg tot maximaal tweemaal 10 mg per dag. Was de laatstgenoemde dosis onvoldoende dan werd overgegaan op insuline.
