Diepe veneuze trombose

Abstract

In the Lancet verscheen onlangs een seminar over diepe veneuze trombose. In dit zeer overzichtelijke seminar worden diverse facetten van de aandoening met speciale aandacht voor nieuwe diagnostische en therapeutische strategieen besproken. Diep veneuze trombose kan zowel spontaan ontstaan als het gevolg zijn van klinische omstandigheden, zoals chirurgische ingreep, trauma, de aanwezigheid van een maligne proces of langdurige bedrust. Bij de therapeutische strategieen wordt dan ook onderscheid gemaakt in (primaire) preventie en curatieve behandeling. Een trombus door klinische omstandigheden is goed te voorkomen door tromboprofylaxe. Bij patienten met een laag risico kan men veelal volstaan met fysische methoden (snelle mobilisatie en/of steunkousen). Bij patienten met een matig tot hoog risico zijn anticoagulantia noodzakelijk. Daarbij hebben laagmoleculaire heparines (bijv. enoxaparine en dalteparine) de voorkeur, ook heparine, vitamine-K-antagonisten en fondaparinux zijn toepasbaar. Diepe veneuze trombose vindt doorgaans in de benen plaats en het beleid is gericht op het voorkomen van het posttrombotisch syndroom en/of embolisatie.
