Abstract

La bacterieHelicobacter pylori est associee a diverses pathologies de severites variables, dont la gastrite chronique, l'ulcere peptique, le lymphome MALT et l'adenocarcinome gastrique. Cette extreme variete de pathologies s'explique, au moins en partie, par l'heterogeneite de la quasiespeceH. pylori. Le mosaicisme, illustre par le gene de la toxine vacuolisante (vacA) et les genes non conserves, tels ceux de l'ilot de pathogenicitecag, contribuent a la diversite genetique du micro-organisme. En outre, des determinants non proteiques, comme le lipopolysaccharide, participent egalement a l'heterogeneite deH. pylori. A l'inverse de ces facteurs qui sont differentiellement exprimes parmi les souches deH. pylori, la mobilite et la forte activite ureasique constituent des traits communs a tous les isolats cliniques. Ces deux proprietes sont indispensables a la colonisation persistante de la muqueuse gastrique. Le present article propose une revue non exhaustive de facteurs de virulence variables et conserves, abordes sous l'angle de la genetique et de la biologie moleculaire. Les analyses moleculaires, qui culminent avec le recent sequencage du genome deH. pylori, apportent certaines reponses et posent de nouvellles questions dans la problematiqueH. pylori.
