Tumeurs stromales gastro-intestinales (GIST)

Abstract

Les tumeurs stromales gastrointestinales (GIST), auparavant considerees comme des tumeurs du muscle lisse et classees comme leiomyomes et leiomyosarcomes, sont maintenant reconnues comme des entites distinctes de premiere importance parmi les tumeurs mesenchymateuses benignes et malignes du tractus gastrointestinal (GI). La caracteristique specifique des GIST est l’expression du proto-oncogene ckit (CD117), un recepteur membranaire de l’activite tyrosine-kinase. La taille de la tumeur et l’activite mitotique peuvent etre predictives du comportement biologique des GIST. L’endoscopie et principalement l’endosonographie avec biopsie-aspiration a l’aiguille fine jouent un role important dans le diagnostic et le suivi. La resection chirurgicale represente le traitement de choix des GIST symptomatiques ou des lesions suspectes de malignite. Les GIST malignes non resecables et metastatiques peuvent beneficier d’une therapie ciblee, grâce a un inhibiteur de la tyrosine-kinase comme le mesylate d’imatinib.
