Le cas rapporte est celui d'un homme de 74 ans presentant des tumeurs superficielles de vessie recidivantes depuis 10 ans, traitees par radiotherapie externe, chimiotherapie et resection endoscopique. Une nouvelle recidive fait instaurer une BCG-therapie intravesicale. Huit mois plus tard, la realisation d'une uretrographie retrograde permet le diagnostic de fistule uretrocutanee, de la jonction uretrobulbaire exterieure a la jonction penoscrotale. Il existe un diverticule sur le trajet fistuleux. L'examen anatomopathologique met en evidence des granulomes specifiques, a centre caseeux.
