A case is reported of an unusual sinographic appearance and negative microbial cultures, indicating a long-term failure of incorporation of a vascular graft by surrounding connective tissue. Possible pathogenic mechanisms include ruptured lymphatic channels or a low-grade immunologic rejection process. In such cases a conservative approach is probably warranted rather than the aggressive measures necessary in cases of perigraft infection.
