Enuresis nocturna und Epilepsie

Abstract

Von 88 mittels EEG untersuchten Bettnassern zeigten 60 ein abnormes Hirnstrombild, davon in 30 Fallen, also in etwa einem Drittel als sicher pathologisch zu verwertende, krampfspezifische Ablaufe. Durch einen Vergleich von Vorgeschichte und Befund mit einer nicht mittels EEG untersuchten Gruppe von 76 Bettnassern konnte eine einseitige Auslese der mit EEG untersuchten Gruppe ausgeschlossen werden. Es wird eine funktionelle Verwandtschaft der Enuresis nocturna mit der Epilepsie in ihrer gemeinsamen Bindung an die Schlafphasen diskutiert und eine Analogie zur Schlaf- und Aufwach-Epilepsie angenommen. Auch die teilweise therapeutische Beeinflusbarkeit der Enuresis nocturna durch Antiepileptika kann — wenn auch nur sehr zuruckhaltend — als Hinweis auf eine solche Verwandtschaft gewertet werden. Die Enuresis nocturna wird wie manche Epilepsieformen als eine Funktionsstorung angesehen, die an eine gestorte vegetative Gestimmtheit in den einzelnen Schlafphasen geknupft ist. Dies steht nicht im Widerspruch mit der Auffassung, das die Enuresis nocturna im allgemeinen eine psychosomatische Storung ist, die ihre Auserungsform ahnlich wie der Pavor nocturnus in einer konstitutionellen oder erworbenen vegetativen Storung und Storung der Schlaf-Wachsteuerung mit paroxysmalem Ablauf findet und damit einem Teil der Falle der Epilepsie nahesteht.
