
The surgical indication for T-OPLL can be classified into three groups: first, myelopathy caused by T-OPLL requires surgical intervention; second, the discontinuous type across the disc space between the rostral and caudal ossification regions requires special care without myelopathy, and immediate surgical intervention if myelopathy develops; and third, the continuous type without myelopathy at the initial visit can be observed with confidence.
