Abstract

Die A. coeliaca entspringt in Hohe von Th 12 oder vom oberen Rand von L 1, je nachdem wie weit sie wahrend der Embryonalentwicklung caudal gewandert ist. Sie besitzt nur einen sehr kurzen Stamm, der teilweise noch durch den Hiatus aorticus zieht und mitten im Plexus coeliacus liegt. Dort, wo die A. coeliaca den Hiatus aorticus verlast, kann sie durch das Ligamentum arcuatum medianum von ausen eingeschnurt werden. Die daraus resultierende klinische Symptomatik ist unter dem Namen „celiac axis syndrom“ [63, 97] bzw. „arcuate ligament encroachment“ bekannt [149, 156]. Unter Bildung des Tripus Halleri teilt sich die A. coeliaca in die A. hepatica communis, A. gastrica sinistra und A. lienalis auf, wobei beim langen Stamm eher eine Bifurkation (49%) und beim kurzen eher eine Trifurkation (25%) der A. coeliaca vorliegen soll. In 9% entspringen die einzelnen Aste teilweise oder ganzlich aus der Aorta, wahrend bei 7% der Patienten einer oder mehrere Hauptaste der A. coeliaca gemeinsam mit der A. mesenterica superior entstehen. Man spricht dann vom Truncus hepatomesentericus [94].
