Among the recent changes in the pathologic staging of patients with melanoma, examination of sentinel lymph nodes (SLN) is one of the most important ones. In addition to a possible survival improvement, the status of the SLN provides crucial prognostic information, to be added to the histologic characteristics of the primary lesion. This chapter reviews the procedure, the histologic evaluation, the contribution of immunohistochemistry, the histologic pitfalls, and its prognostic significance (with special emphasis on tumor burden).
