Rotational knee instability following anatomic anterior cruciate ligament (ACL) reconstruction has warranted a new look at the anterolateral structures of the knee and their biomechanical behavior. Although the anatomy of the knee has not changed, controversy has arisen from the recent description of a novel anterolateral ligament (ALL). Whether there is a true ligament or not is not a key part to understanding and describing the biomechanical behavior of the anterolateral knee complex as a whole. Most cadaveric studies focus on the whole complex and do not clearly differentiate between ligament and capsule.
