Abstract

Actuellement environ 10 a 15 % des accouchements par voie basse necessitent une aide instrumentale, ventouse, forceps ou plus rarement spatules. Des complications chez le nouveau-ne peuvent etre observees, notamment au niveau de l’extremite cephalique. Elles sont rares et le plus souvent benignes. Neanmoins, lorsque l’extraction fœtale a ete difficile, des complications plus graves peuvent survenir, comme l’hematome extensif du cuir chevelu, certaines hemorragies intracrâniennes ou des fractures du crâne. Elles dependent pour une part de l’instrument utilise mais aussi du contexte obstetrical et notamment du role deletere d’une asphyxie per-partum. Cette derniere expose le nouveau-ne au risque d’encephalopathie anoxo-ischemique. Dans ces circonstances, le pediatre devra etre a meme de prendre en charge des la naissance ce nouveau-ne, de depister et de traiter des complications qui pourraient apparaitre secondairement. Une information claire devra etre donnee aux parents quant aux differents mecanismes lesionnels.
