As the editorial team of Liver Transplantation transitions it is an opportunity for reflection as well as gratitude. Five years ago hepatitis C (HCV) management had already been dramatically altered by new all oral treatment regimens with the threat of viral recurrence posttransplant already receding. Timing of HCV therapy now reflects considerations which include use of an organ from an HCV-positive donor even in HCV-naive recipients 1 .
