Key Points Transcatheter mitral valve replacement (TMVR) appears to have better early survival compared to surgical mitral valve replacement (SMVR). Over the long‐term, TMVR has attenuated survival compared to SMVR and this attributed to advanced age, chronic lung disease, more advanced heart failure and pulmonary hypertension. Low rates of mortality for TMVR are likely linked to the iterative improvement in techniques to use transseptal access for implantation.
