Abstract

A56-year-oldwomanpresentedwitha several-yearhistoryofnasal obstructive symptoms and anosmia. Despite previous courses of fluticasonepropionate,sheexperiencedno improvement.Shehad noassociatedpain, rhinorrhea,headaches, changes invision,or epistaxis, and her medical history was negative for either allergic rhinitis or chronic rhinosinusitis. Findings from the physical examinationwerenotable forbilateral, obstructingnasalmassesarising from the superior nasal cavity (Figure, A). She was placed on a 14-day course of oral prednisone, 60 mg perday,witha5-day taper, inaddition to fluticasonenasal spray.The patient reported some initial improvement of her symptomswhile taking prednisone, albeit without complete resolution, with return of her symptoms after cessation of systemic steroids. A computed tomographic (CT) scan was obtained and demonstrated polypoid masses within the olfactory clefts bilaterally with marked demineralization of the cribriform plates (Figure, B). Magnetic resonance imaging (MRI) demonstrated soft-tissue changes at the crista galli extending to the margin of the cribriform plate without dural enhancement or intracranial involvement. The patient was subsequently taken to the operating room for abiopsyandbilateral subtotal resectionof themasses.Hematoxylineosin stain of the specimen at low (Figure, C) and high (Figure, D) magnifications revealedproliferationofglandular structureswithciliatedepitheliumandabundant goblet cells. Glandswere in continuity with the surface epithelium, invaginating into the submucosa without evidence of epithelial dysplasia or atypia. Without further treatment, the patient reported complete resolution of her symptoms 1 year postoperatively. What is your diagnosis? A B
