Antral and nasal vestibular recurrence of retinoblastoma is reported in a 10-year-old girl 6½ years after enucleation of the ipsilateral eye (8½ years after enucleation of the contralateral eye). There was no evidence of visceral or osseous metastasis. The treatment was radical maxillectomy. Spread to the antrum was apparently bloodborne. The tumor cells then remained in a state of "suspended viability" for a period of years before resuming rapid proliferation and becoming clinically evident. Continued, careful long-term follow-up is indicated in cases of retinoblastoma because of rare late recurrences.
