Azitromicin u trudnoći i dojenju

Abstract

Kako je 50% trudnoća neplanirano, a u manje od 50% slučajeva trudnoća se otkrije prije 4. tjedna koncepcije, važno je za svaki lijek koji se široko primjenjuje da je siguran u trudnoći. Azitromicin po svojem indikacijskom području svakako ima široku primjenu i sigurnost u trudnoći znači važnu prednost osobito kada se usporedi s antimikrobnim lijekovima koji se rabe u istim indikacijama. Npr. doksiciklin u liječenju infekcija uzrokovanih klamidijama, fl uorirani kinoloni u liječenju infekcija respiratornog sustava, klaritromicin u liječenju infekcija respiratornog sustava. Azitromicin prelazi placentarnu barijeru, ali u plazmi ploda postiže tek 7% koncentracije u plazmi majke. Kao i za većinu ostalih antimikrobnih lijekova i za azitromicin nedostaju kontrolirane studije sigurnosti i neškodljivosti provedene u trudnica, no na temelju do sada dostupnih podataka, kao i mehanizma djelovanja azitromicina prema FDA nosi kategoriju B. Kako je do danas azitromicin primio značajan broj trudnica, može se pretpostaviti da je sigurnost azitromicina u trudnoći veća danas nego u vrijeme kada je dobio kategoriju B od FDA. Azitromicin prelazi u majčino mlijeko, no nema indicija da je škodljiv za dojenče. U slučaju potrebe za azitromicinom u dojilje, nije potrebno prekinuti s dojenjem.
