FACTORES ASOCIADOS AL ABANDONO DE LA LACTANCIA MATERNA EXCLUSIVA

Abstract

Introducción: la lactancia materna (LM) es uno de los factores que más contribuye con la salud y la alimentación de los niños, especialmente si se ofrece en forma exclusiva hasta el sexto mes. Objetivo: determinar la tasa de abandono de la lactancia materna exclusiva (LME) y sus factores asociados en Bucaramanga. Metodología: estudio de cohorte no concurrente con nacimientos ocurridos entre enero y agosto de 2006 en la ciudad de Bucaramanga, Colombia, seleccionados al azar. Se realizó un análisis de supervivencia. Resultados: tasa de incidencia de abandono de la LME de 362,6 (IC 95% 315,5 - 416,8) por cada 1000 meses-persona de seguimiento. Se encontraron asociados al abandono de la LME: los conocimientos deficientes sobre la LM, el uso de biberón, el estrato socioeconómico bajo, la información recibida durante la estancia hospitalaria. Conclusiones: se presentó una alta incidencia de abandono de la LME, identificando varios factores asociados al abandono que permiten determinar la población vulnerable para posibles intervenciones. Introduction: breastfeeding (BF) is one of the factors which contributes the most with health and the feeding of children, especially if it is offered in exclusive form until the sixth month. Objective: To determine the rate of discontinuation of exclusive breastfeeding (EBF) and the factors associated in Bucaramanga. Methodology: Study of nonrecurring cohort with births occurring between January and August 2006 in the city of Bucaramanga, chosen at random. A survival analysis was carried out. Results: rate of incidence of EBF discontinuation 362.6 (IC 95% 315.5 - 416.8) per every 1000 month-person of follow-up. The following factors were associated to the EBF discontinuation: deficient knowledge about BF, the use of baby bottles, the low socioeconomic stratum, and the information received during the hospital stay. Conclusions: a high incidence of EBF discontinuation during the first six months was presented, identifying some associated factors to the discontinuation which allow the determination of vulnerable population for possible intervention. Introdução: A lactação materna (LM) é um dos fatores que mais contribuem com a saúde e a alimentação das crianças, especialmente se oferece duma exclusiva até o sexto mês. Objetivo: determinar a taxa de abandono da lactação materna exclusiva (LME) e seus fatores associados em Bucaramanga (cidade localizada ao nordeste da Colômbia). Metodologia: estudo de cohorte (consiste num estudo epidemiológico, observacional) não concorrente com nascimentos ocorridos entre janeiro em agosto de 2006 na cidade de Bucaramanga, Colômbia, selecionada ao azar. Realizou-se um analise de supervivência. Resultados: taxa de incidência de abandono da LME de 362,6 (IC 95% 315,5 - 416,8) por cada 1000 meses-pessoa de seguimento. Encontraram-se associados ao abandono da LME: os conhecimentos deficientes sobre a LM; o uso de mamadeira, o estrato socioeconômico baixo, a informação recebida durante a estância hospitalaria. Conclusões: Se presenteou uma alta incidência de abandono da LME, identificando vários fatores associados ao abandono que permitem determinar a povoação vulnerável para possíveis intervenções.
