Döendets moral : Om moraliskt lidande i vård vid livets slut

Abstract

I den här vetenskapliga essän undersöker jag existentiella dimensioner av lidande i vården av patienter i livets slut. Essän är både metod och form. I mitt arbete som sjuksköterska i palliativ vård ser jag sällan att den moraliska dimensionen av lidandet behandlas i vården av och akademiska eller professionella diskussioner om existentiellt lidande. Jag reflekterar över den med utgångspunkt i situationer hämtade ur min yrkespraktik. Essän är strukturerad genom tre huvudteman: Smärta, sömn och död. Genom Marcia Sá Cavalcante Schuback och Viktor Frankl lyfts smärtans och lidandets ontologiska betydelse fram. Med Schuback ifrågasätts några etablerade föreställningar om smärta som jag misstänker påverkar såväl vår uppfattning som upplevelse av den. Det gäller bland annat föreställningen om smärtans själviskhet och föreställningen om att man kan med-lida på distans, genom icke-känsla. I detta sammanhang behandlar jag frågan om smärtans mätbarhet och vad mätningen i sig kan få för konsekvenser. Med Schubacks läsning av Aristoteles utreder jag sömnens likhet med döden och hur sömnen kan förstås som någonting annat än världsfrånvaro. Genom Arendt närmar jag det politiska med det moraliska och jag undersöker hur döden och döendet kan förstås i dessa termer. Jag frågar mig om den döende kan avkrävas moraliskt ansvar och för den diskussionen till sin spets med hjälp av självmordet. Jag utvecklar denna diskussion med Beauvoir, Aristoteles och Frankl. Frågan om vilken roll moral har för upplevelsen av mening är genomgående i hela essän.

In this scientific essay I will examine existential dimensions of pain in end of life care. The essay is both the method and the form. As a nurse in palliative care I rarely see the moral dimension of pain being acknowledged in the care of and in academic and professional discussions about existential pain. I make this reflexion based on situations from my work practice. The essay is structured in three themes: Pain, Sleep and Death. Through Marcia Sá Cavalcante Schuback and Viktor Frankl the ontological aspects of pain will be brought into light With Schuback some established notions on pain is questioned. These are notions that affect the perception of pain. It is the notion about pain as selfish and the idea that the feeling of compassion can be felt on a distance without ones own feeling being involved. In this context I will discuss the measurability of pain and what consequences there can be from measuring it. With Schubacks reading of Aristotle I will examine the similarities between death and sleep and how death can be seen as something else then absence from the world. Through Arendt I see a resemblance between the concept of moral and the political and examine how death and dying can be understood within this concept. I ask if the dying person can be required to take moral responsibility. This discussion evolves around the concept of suicide through the thinking of Beauvoir, Aristotle and Frankl. The question of the role of moral in the experience of meaning runs through the essay as a whole.
