Riscul cardiovascular și riscul de apnee în somn la persoanele cu diabet zaharat tip 2

Abstract

Pandemia globală a bolilor metabolice asociate frecvent cu sindromul de apnee în somn SAS reprezintă un factor de risc major pentru mortalitatea prin boli cardiovasculare. Respectiv, este primordială stratificarea riscului pentru sindromul de apnee în somn, precum și a riscului pentru bolile cardiovasculare la toate persoanele cu diabet zaharat. Studiul transversal descriptiv a inclus 150 de persoane cu DZ tip 2, internate în secția Endocrinologie al Spitalului Clinic Republican bdquo;Timofei Moșneagardquo; în perioada septembrie-noiembrie 2023. Pentru stratificarea riscului pentru SAS a fost utilizat chestionarul STOP-BANG, iar pentru evaluarea riscului cardiovascular ndash; sistemul SCORE 2-Diabetes. Se estimează că 44% dintre persoanele cu DZ tip 2 prezintă risc intermediar pentru SAS; 34% ndash; risc înalt și doar 22% prezintă risc redus, iar conform chestionarului SCORE 2-Diabetes: 0,7% dintre persoanele cu DZ tip 2 studiate prezintă risc scăzut; 6,7% risc moderat; 21,3% risc înalt și 71,3% risc foarte înalt pentru patologie cardiovasculară. Majoritatea persoanelor cu DZ tip 2 prezintă risc intermediar și înalt pentru SAS, precum și risc foarte înalt pentru evenimente cardiovasculare, iar excesul de greutate raportat prin IMC și CA a fost mai mare în rândul persoanelor cu DZ de tip 2 și risc înalt pentru SAS. Totodată, se observă că majoritatea pacienților cu risc înalt și foarte înalt CV prezintă risc crescut pentru SAS.

Глобальная пандемия метаболических заболеваний, обычно связанных с апноэ во сне, является основным фактором риска смертности от сердечно-сосудистых заболеваний. Соответственно, необходима стратификация риска развития синдрома апноэ во сне у всех пациентов с диабетом. В описательное поперечное исследование были включены 150 пациентов с СД 2 типа, госпитализированных в эндокринологическое отделение Республиканской клинической больницы имени Тимофея Мошняги в период с сентября по ноябрь 2023 года. Для стратификации риска по САС и сердечно-сосудистым заболеваниям использовался опросник STOP-BANG. риск ndash; система SCORE 2. Установлено, что 44% больных СД 2 типа относятся к группе промежуточного риска по САС, из них 34% ndash; к высокому и только 22% ndash; к низкому риску. А по системе SCORE 2-диабет: 0,7% больных СД 2 типа представляют низкий риск; 6,7% умеренный риск; 21,3% высокий риск и 71,3% очень высокий риск. Большинство пациентов с СД 2 типа имеют промежуточный и высокий риск развития САС, а также очень высокий риск сердечно-сосудистых событий. А избыточный вес, о котором сообщают ИМТ и CA, выше среди пациентов с СД 2 типа и высоким риском развития САС. Дополнительно отмечается, что большинство пациентов с высоким и очень высоким сердечно-сосудистым риском находятся в группе повышенного риска развития САС

The global pandemic of metabolic diseases commonly associated with sleep apnea is a major risk factor for cardiovascular disease mortality. Accordingly, risk stratification for sleep apnea syndrome in all patients with diabetes is essential. The descriptive study included 150 patients with DM type 2, hospitalized in the Endocrinology department of the bdquo;Timofei Moșneagardquo; Republican Clinical Hospital between September and November 2023. The STOP-BANG questionnaire was used for risk stratification for SAS, and for cardiovascular risk - the system SCORE 2-Diabetes. It is determined that 44% of patients DM2 are at intermediate risk for SAS, 34% of them are at high risk and only 22% are at low risk. And according to the SCORE 2-diabetes system: 0.7% of patients with DM type 2 present a low risk; 6,7% moderate risk; 21,3% high risk and 71,3% very high risk. Most patients with type 2 DM present intermediate and high risk for SAS, as well as very high risk for cardiovascular events. And overweight reported by BMI and CA is higher among patients with type 2 DM and high risk for SAS. Additionally, it is noted that most patients with high and very high CV risk are at increased risk for SAS.
