Abstract

Гиперхолестеринемия ГХ представляет собой гиперлипидемию, напрямую связанную с повышенным риском сердечно-сосудистых событий ССС, преимущественно коронарного генеза. Ранняя диагностика и верная терапевтическая стратегия, разработанная для достижения целевого уровня холестерина ЛПНП, по-прежнему считаются главной задачей и может быть достигнута в контексте этапной терапии и междисциплинарного менеджмента. Приводим случай пациента 3-х лет, поступившего в Кардиологическую клинику Института Матери и Ребёнка с подозрением на нарушения липидного обмена, возможно, первичного генеза общий холестерин, без терапии - 295,05 мг/дл 7,63 ммоль/L, с небольшим нарушением общего состояния, и жалобами на прекардиальные боли. Поскольку в липидном профиле преобладали гипер-ЛПНП и гипер-ХТ, в связи с положительными результатами реверс-скрининга рассматривался диагноз семейной гиперхолестеринемии без поражения сердечно-сосудистой системы. В связи с высоким профилем риска при выписке было принято решение о связи между здоровым, сбалансированным питанием CHILD-1 - ограничение потребления насыщенных жиров, физической активностью. В данном случае гиполипидемическая фармакотерапия была отсроченна из-за юнного возраста пациента, не включенному в диапазон для начала лечения.

Hypercholesterolemia HC represents hyperlipidemia, directly and gradually associated with an increased risk of cardiovascular events CV, predominantly of coronary origin. Therefore, early diagnosis and the therapeutic strategy developed to reach the LDL-c target remain fundamental and can be achieved in the context of staged therapy and multidisciplinary involvement. We present the case of a 3-year-old patient, admitted to the Cardiology Clinic, PMHI IMC, with the suspicion of primary lipid metabolism disorder total cholesterol, outside of treatment - 295.05 mg/ dL 7.63 mmol/L, with a slightly influenced general condition, presenting a precordial discomfort. Since a lipid profile dominated by hyper-LDL-C, and hyper-TC was recorded, in association with the positive results of the reverse screening, the diagnosis of familial hypercholesterolemia without cardiovascular involvement was considered. Due to the high-risk profile, it was decided at discharge for the association between a healthy, balanced diet CHILD-1 - the limitation of saturated fats intake and physical activity. In this case, hypolipidemic pharmacotherapy was delayed due to the young patient#39;s age.
