Abstract

În pofida unui număr mare de cercetări realizate atât de autorii autohtoni, cât și de cei străini, axate pe studiul aspectelor biologice moleculare ale cancerului endometrial, în prezent, niciunul dintre markerii tumorali nu este recomandat pentru o utilizare clinică la scară largă în cazul cancerului de corp uterin, deoarece nu există suficiente informații pentru introducerea acestora în practica clinică. Conform recomandărilor actuale ale ESMO, pentru managementul pacientelor cu tumori endometriale, tactica de tratament este determinată de rezultatele stratificării riscului. În ceea ce priveşte cancerul de CU, sunt descriși un număr mare de factori de prognostic, fapt care creează anumite dificultăți în aplicarea acestora în practica clinică. Majoritatea factorilor sunt morfologici, iar informațiile despre aceștia sunt obținute în baza unui examen histologic standard. Datele respective sunt utilizate pentru a evalua riscul metastazelor limfoganglionare, a face prognoze în legătură cu evoluția bolii și viața pacientelor cu CU și pentru a planifica tratamentul postoperator. Studiul factorilor clinici, dar şi a celor morfologici de prognostic în cadrul anumitor etape, în special în cele timpurii, au fost deja realizate de către alți autori. Totuşi, publicațiile axate pe analiza unei game largi de factori clinici și morfologici și markeri tumorali, se întâlnesc rar.

Despite the large number of works, both by domestic and foreign authors, devoted to the study of the molecular biological aspects of endometrial cancer, currently none of the tumor markers is recommended for wide clinical use în endometrial cancer EC, since there is not enough information for their introduction into clinical practice. According to current ESMO guidelines for the management of patients with endometrial tumors, treatment tactics are determined based on the results of risk stratification. With EC, a large number of prognostic factors are described, which creates certain difficulties in their application in clinical practice. The vast majority of these factors are morphological, and information about them is obtained by standard histological examination. This information is used to assess the risk of lymph node metastases, to predict the progression and life of patients with EC, and to plan after surgical treatment. The study of clinical as well as morphological factors of prognosis within the individual stages, especially the early ones, has already been carried out by other authors. However, publications devoted to the analysis of a wide range of clinical and morphological factors and tumor markers are not common. Of great interest is the study of risk factors for individual variants of EC progression.

Несмотря на большое количество работ, как отечественных, так и зарубежных авторов, посвященных изучению молекулярно-биологических аспектов рака эндометрия, в настоящее время к широкому клиническому использованию при раке тела матки РТМ не рекомендован ни один из опухолевых маркеров, поскольку не достаточно информации для их внедрения в клиническую практику. Согласно современным рекомендациям laquo;ESMOraquo; по ведению пациенток с опухолями эндометрия, тактика лечения определяется по результатам стратификации риска. При РТМ описано большое число прогностических факторов, что создает определенные трудности при их применении в клинической практике. Подавляющее большинство этих факторов морфологические, а информацию о них получают при стандартном гистологическом исследовании. Эту информацию используют для оценки риска метастазов в лимфатических узлах,прогноза прогрессирования и жизни больных РТМ, а также для планирования после операционного лечения. Изучение клинических, а также морфологических факторов прогноза в пределах отдельных стадий, особенно ранних, уже проводилось другими авторами . Однако публикации,посвященные анализу целого спектра клинико-морфологических факторов и опухолевых маркеров, встречаются нечасто. Большой интерес представляет изучение факторов риска отдельных вариантов прогрессирования РТМ.
